Cancellation and Amendment Policy
CosMediTour (Consulting) Pty Ltd (“CosMediTour”) ACN 115 250 452 of Level 2, Medical Precinct, Oasis Centre, Victoria Avenue, Broadbeach Queensland Australia, and “the Client” acknowledges and accepts the Cancellation and Amendment Policy as follows:
1. Deposit
1.1 You will be required to pay a deposit when booking. All deposits are non-refundable. Final payment is required no later than 6 weeks prior to your scheduled surgery date.
1.2 The deposit may be applied as a once-off contribution towards a procedure valued greater than $4,990 AUD booked through CosMediTour Australia or CosMediSmile with a surgery or treatment date within 24 months from the invoice issue date.
1.3 The deposit secures your preliminary quote for a validity period of 12-months from the date of deposit payment. Surgery scheduled outside of the 12-month validity period is subject to any applicable price increases.
1.4 Surgery Deposits are non-refundable in the event that you are unable to attend your booked and scheduled surgery date, due to a government-imposed travel restriction, government-mandated medical restrictions, or personal reasons relating or not relating to COVID-19. Your Surgery Deposit can be used as a credit towards a new CosMediTour surgery date within 24 months from the invoice issue date.
2. Amendment and Cancellation Fees
2.1 Amendment or Cancellation by the Client more than 30 days prior to the Confirmed Surgery Date will incur an Amendment fee of $150 or a Cancellation Fee of $350.
2.2 Amendment or Cancellation by the Client within 30 days of the Confirmed Surgery Date will incur an Amendment fee of $250 or a Cancellation Fee of $500.
2.3 Amendment or Cancellation by the Client within 7 days of the Confirmed Surgery Date will incur an Amendment fee of $300 or a Cancellation Fee of $1000.
2.4 Cancellation of the Surgery by the Surgeon at the time of Consultation, or after the Consultation and prior to the Confirmed Surgery, and as a result of the Client’s non-disclosure of medical history, medications or non-compliance with pre-surgery CosMediTour Guidelines and Conditions will incur a $2000 Cancellation Fee, an Administration Fee of $500 will apply, and any costs incurred including but not limited to all medical tests at the hospital, surgeon consultation, hospital inpatient accommodation, and all transfers.
2.5 If the Surgeon or the Medical Team, including the Anaesthetist, cancels the Surgery in the interests of patient safety due to increased patient risk, and not as a result of the Client’s non-disclosure of medical history, medications or non-compliance with pre-surgery CosMediTour Guidelines and Conditions, no Cancellation Fee will apply, however, an Administration Fee of $500 will apply, and any costs incurred including but not limited to all medical tests at the hospital, surgeon consultation, hospital inpatient accommodation, and all transfers.
2.6 Cancellation of the Surgery by the Client at the time of Consultation with the Surgeon, or after the Consultation with the Surgeon and prior to the Confirmed Surgery will incur a $1000 Cancellation Fee, an Administration Fee of $500 will apply, and any costs incurred including but not limited to all medical tests at the hospital, surgeon consultation, hospital inpatient accommodation, and all transfers.
2.7 Cancelled Accommodation bookings within 30 days of the Client’s scheduled check-in date will also incur hotel cancellation fees, which can be up to 100% of the cost of the Accommodation.
2.8 The Deposit cannot be applied to Amendment and Cancellation Fees.
2.9 We strongly recommend that you take out a suitable Travel Insurance Policy with appropriate coverage for Cancellation Fees.
3. Finance Application Denied
3.1 In the event that you can provide evidence of being denied finance through a third party finance company or bank, within 10 business days of paying your deposit, you will be entitled to a refund of your Booking Deposit less an administrative cancellation fee of $150.
4. Processing Refunds
4.1 Bank account details must be provided to CosMediTour in writing by email. Once bank account details are received, refunds may take up to 1 week to process. Providing wrong account details for a refund will incur a $25 AUD re-processing fee.
5. COVID-19
5.1 In the event that you are unable to travel due to Government-imposed travel restrictions or government-mandated medical restrictions, your surgery date can be postponed without any administration or cancellation fee. Please note, if government-sanctioned restrictions are already in place at the time of booking your surgery and restrictions continue necessitating amendment or cancellation of your surgery booking, CosMediTour will require a minimum of 30 days notice to amend or cancel the surgery date without incurring a fee.
5.2 In the event that Government restrictions are imposed within 30 days of your scheduled surgery date, you will not incur an administration or cancellation fee.
5.3 The administration or cancellation fee is $150 AUD.
5.4 CosMediTour is not responsible for advising of current applicable government travel restrictions and all clients must ensure they research and keep up to date with ongoing travel restrictions, by accessing the applicable government resources.
